If you also bill clients for expenses, keep those numbers separate from your hours worked. Attach an expense sheet to your invoice or list expenses separately on your standard invoice. This will help you out come tax time, since our friends at the IRS like to see revenue and expense figures in their own little columns.

You can be as general or specific as you'd like when describing the services performed. You'll need to make this call yourself, taking into consideration each client's preferences. Regardless, each invoice should clearly state hours worked and your hourly fee or the charge for completing a job, whichever applies.
