I highly recommend www.DylanDesignLab.com for your website creation. He created my website from scratch (www.weddingsingermn.com) but also will design websites from templates for very cheap. I have seen some of his stuff designed from templates and he does a really good job of turning the template into something amazing that works for your company.
He also owns a hosting company and can get you set up with hosting, domain names, email for your company, etc. That site is: www.HostRail.com
