Does anyone have any experience setting up one of these facilities?
Associated setup costs?
Insurance?
Any help would be appreciated.

I would suggest seeing if there are any franchise systems that already provide this service and ask them your questions. Some people are a good fit for franchise systems and some are not. Whether you decide to buy into that franchise system or not, you could get your questions answered.
Also, find an indoor rock climbing gym outside of your area (about 100 miles away). Go visit as a customer and casually start asking questions.
